The Organization Evidence Pack 2.3C.4-3

Application Sidebar — Unit 2.3C.4

By Charisma V.

Published March 17, 2026 — Revised March 19, 2026

The organization evidence pack assembles the artifacts produced by Sub-Arc 3C's four units into a minimal, privacy-preserving packet that proves the organization's practice is real. The governing principle: processes public by default; persons private by default.

The pack is not a compliance filing. It is not a marketing document. It is structural evidence that the institution has designed its governance (2.3C.1), restrained its scope (2.3C.2), encoded and audited its protocols (2.3C.3), detected its incentive failures and recovered from its incidents (2.3C.4) — and can prove all of it without exposing the people who operate within the structure.

Pack Contents

From Answerability (2.3C.1)

  • Institutional charter — current version with amendment history. Documents mandate, authorities, limits, accountability ladders, and sunset provisions.
  • Accountability ladder — who reports to whom, with what authority to correct, at what cadence. The structural map of institutional oversight.
  • Rights boundary tests — results of audits evaluating whether declared rights are structurally enforced. Gaps documented with remediation plans.
  • Exception ledger — public record of times the institution violated its own rules, who authorized the exception, why, and what review followed.

From Restraint (2.3C.2)

  • Complexity budget assessments — documented pre-commitment evaluations for significant institutional additions. Load, displacement, and retirement conditions recorded.
  • Scope-cap audit results — commitment inventory with classifications (load-bearing, legacy, institutional fiction) and load-to-capacity ratio. Sunset decisions documented.

From Encoded Objective Law (2.3C.3)

  • Enforce-in-code decisions — documented assessments for each critical decision domain: what was evaluated, what criteria applied, what encoding mechanism was chosen, and why.
  • Data governance inventory — data map, access rules, retention schedules, and privacy impact assessments.
  • Finance controls documentation — segregation of duties, approval thresholds, capped discretion levels, and append-only ledger architecture.
  • Change control records — protocol changes with proposal, review, approval, and rollback documentation.

From Moral-Hazard and Resilience (2.3C.4)

  • Hazard heatmaps — quarterly assessments going back at least twelve months, showing risk trends across decision domains.
  • Counter-gaming installs — specific structural counters deployed against identified hazards, with effectiveness tracking.
  • Incident postmortems — three-part narratives (what happened, what was learned, what changed) with recurrence tracking. Process details public; person-specific details protected.
  • Vendor terms and exit conditions — agreements, portability specifications, escrow arrangements, and exit cost documentation.
  • Board design and oversight structure — polycentric distribution of oversight, rotation schedules, independent member requirements.

The Privacy Boundary

The evidence pack includes structural artifacts — charters, policies, audit results, hazard heatmaps, postmortem summaries, vendor terms. It does not include person-specific data: individual performance records, dissent filed through whistle-safe channels, personal circumstances cited in exception requests, or the identities of people who triggered investigations.

The distinction is the one RO applies everywhere: processes are subject to structural accountability; persons are subject to structural protection. An organization that publishes its hazard heatmap but protects the identity of the person who reported the hazard has maintained the boundary. An organization that demands personal exposure as the price of institutional transparency has violated it.

Maintenance Cadence

The evidence pack is not assembled once. It is maintained on a defined schedule.

Monthly: Update change control records, access audit results, and exception ledger entries.

Quarterly: Update hazard heatmap, add new postmortems, review counter-gaming effectiveness, run scope-cap audit against current commitments.

Annually: Full pack review — is every section current? Have structural changes been documented? Have sunset decisions been executed? Has the charter been renewed?

On trigger: Any Severity 1 incident triggers immediate pack update with the postmortem and remediation documentation. Any charter amendment triggers immediate update. Any significant vendor change triggers exit-term and portability review.

An evidence pack that is not maintained is a snapshot of a moment. It proves what the institution was, not what it is.

Assembly and Access

Assembly. The evidence pack is the responsibility of a named person or role — typically the governance lead or chief compliance officer. Not "everyone." Not "the team." One person accountable for completeness, currency, and accuracy.

Internal access. The pack is available to governance bodies (board, oversight committees), leadership, and — for non-sensitive sections — to all people in the organization. An institution that hides its evidence pack from its own people has identified something that needs structural correction, not concealment.

External access. Sections of the pack may be shared with external parties — regulators, auditors, prospective partners — on a selective disclosure basis. Share the minimum needed for the stated purpose, consistent with the privacy boundary.

Integration Points

  • Individual evidence pack (2.3A.3-3): The individual-level pack assembles personal practice artifacts. The organization pack assembles institutional artifacts. The principle is the same at both scales: prove practice without performative exposure.
  • Team evidence pack (2.3B.3-3): The team-level pack sits between individual and organizational scales. The organization pack incorporates team-level artifacts where they bear on institutional governance.
  • Enterprise evidence pack (2.3D.3-4): The enterprise-level pack will incorporate organizational evidence packs from multiple organizations. The closing unit (2.3D.4) synthesizes the evidence-pack concept across all four levels.

Change Log
  • 19 Mar 26 — Review-revise pass (Charisma). Removed type declaration from H1 title. Replaced position-label subtitle with descriptive. Changed --- to * * *. Converted all-caps headings to title case. Restructured from flat inventory list to organized sections by source unit (2.3C.1–2.3C.4). Added privacy boundary section per RO's "processes public, persons private" principle. Added maintenance cadence with monthly/quarterly/annual/trigger schedule. Added assembly and access section with selective disclosure guidance. Added cross-scale integration points connecting to individual (2.3A.3-3), team (2.3B.3-3), and enterprise (2.3D.3-4) evidence packs. Lowercased terms per Capitalization Rules.
  • 17 Mar 26 — Initial draft.
